Case Theme:
This is a case about the failure to timely diagnose and treat phlebitis, thrombophlebitis and extravasation, in an intravenous site after the administration of Anzamet for post-operative nausea. This failure resulted in Compartment Syndrome, an 8 day hospital stay and a residual claw deformity of her left hand.
Opinion:
In my opinion this case does have merit due to inconsistent charting, inadequate charting and no follow up information given to Mrs. C. regarding her left hand on discharge.
Recommendations:
I would recommend:
1. Research Anzamet
2. Review standards of care for PACU Nurses, OR Nurses, ASU Nurses and IV extravasation policies
3. Thorough explanation of Phlebitis, Thrombophlebitis, Extravasation, Compartment Syndrome and Fasciotomy.
4. Write a brief to moderate report with chronological timeline included.
5. Possibly interview Mrs. C. as parts of the medical records are missing and inconsistent.
6. Assist in locating expert witnesses.
